We focus on patients who are transitioning from an acute care setting and/or patients who despite an acute episode are minimally engaged with healthcare generally. Our work is 100% telehealth, which allows us to be more proactive and anticipatory, as well as more rapidly responsive to changing patient conditions, compared to in-home visits.
