We are hiring a Remote Care Manager (LPN) to support patients through meaningful, ongoing care—with the time to build real relationships, not just quick interactions. If you enjoy connecting with patients, educating them, and helping them navigate their healthcare, this role is for you.
This is a fully remote position with consistent hours, strong team support, and a monthly bonus opportunity for high performers.
What You’ll Do
• Speak with patients daily via phone to provide support and guidance
• Help patients understand their care plans and medications
• Coordinate follow-ups with providers and specialists
• Track patient progress and ensure no one falls through the cracks
• Build strong relationships with patients who rely on your support
• Document patient interactions in EHR systems
