What You'll Do
• Deliver skilled nursing care to patients in their homes, guided by an experienced RN supervisor.
• Collaborate with interdisciplinary team members for quality, coordinated care.
• Observe, report, and document changes in patient status and help implement care plans.
• Participate in patient education, medication review, and ongoing care assessments.
• Prepare progress notes and ensure all documentation meets regulatory requirements.
What We’re Looking For
• Texas Licensed CNA
• Reliable car, valid Texas driver’s license, and current auto insurance.
• Current CPR certification, negative TB screen, and Hepatitis profile.
• Familiarity with Kinnser/WellSky preferred but not required.
• Team player with great communication skills, adaptability, and reliability.
• Ability to lift up to 50 pounds and perform regular standing/walking as needed.
