Experienced Case Management Coordinator - Transforming Healthcare on Chicago's South Side

About the Role As a Case Management Coordinator, you'll play a vital role in facilitating healthcare outcomes for our members. You'll work closely with case managers, supervisors, Medical Directors, and other healthcare professionals to provide care coordination, support, and education to members in need. This is a field-based position that requires 50-75% travel to meet members face-to-face on the South Side of Chicago. Key Responsibilities: Conduct comprehensive evaluations of referred members' needs and eligibility using care management tools and data review. Develop and implement care plans, monitoring progress and making adjustments as needed. Collaborate with case managers, supervisors, and Medical Directors to overcome barriers to meeting member goals and objectives. Identify and escalate quality of care issues through established channels. Utilize negotiation and motivational interviewing skills to secure necessary services and promote lifestyle changes. Provide coaching, information, and support to empower members to make informed healthcare decisions. Ensure compliance with regulatory and accreditation guidelines, as well as company policies and procedures. Essential Qualifications To succeed in this role, you'll need: A minimum of 2 years of experience in behavioral health, social services, or human services. A minimum of 2 years of case management experience. Willingness and ability to travel 50-75% of the time to meet members face-to-face on the South Side of Chicago. Residency in one of the applicable zip codes: 60615, 60617, 60619, 60620, 60621, 60628, 60633, 60636, 60637, 60643, 60649, 60652, and/or 60655. Microsoft Office experience (Outlook, Excel). A Bachelor's Degree or non-licensed master's level clinician in behavioral health or human services (psychology, social work, marriage and family therapy, counseling). Preferred Qualifications While not required, the following qualifications can enhance your candidacy: Discharge planning experience. Managed care experience.
