Behavioral Health Care Management Clinician (remote in Oregon)

About the position Responsibilities • Document and present coherent clinical assessments and case formulations. • Develop individualized plans of care. • Teach and provide telephonic support for members and their support systems. • Coordinate with providers at all levels of care. • Facilitate communication between members and their support systems. • Participate in interdisciplinary or multidisciplinary team meetings associated with member care. • Provide technical assistance and training to community partners regarding programs and systems of care. • Perform cross coverage when required, fulfilling all duties of the Utilization Management Integrated Services Coordinator. Requirements • 5 years' experience in mental health. • Knowledge of DSM criteria for mental health and substance dependence/abuse diagnoses. • Experience with programs such as EASA, IIBHT, WRAP, or coordinating between levels of care with hospitals or DHS. • Ability to conduct psychosocial assessments and formulate multiaxial DSM impressions. • Strong communication skills to present, recommend, and listen effectively. • Proficient in Microsoft Office software and/or Google Suites. • Strong planning and organizational skills. • Master's degree in social work, psychology, sociology, or a closely allied field. • Licensed Clinical Social Worker or equivalent Oregon licensed healthcare profession. Nice-to-haves • Bilingual in Spanish preferred.
