Admissions RN (Registered Nurse) β In-Home Intake & Assessment

Responsibilities
β’ Complete Start of Care (SOC) admissions in patientsβ homes.
β’ Conduct detailed clinical assessments and OASIS documentation.
β’ Initiate care plans and ensure a smooth transition into services.
β’ Educate patients and families on what to expect moving forward.
β’ Coordinate with the care team to set patients up for success.
Requirements
β’ Active Minnesota RN license.
β’ Strong time management and independent work style.
β’ Comfortable working in a field-based, patient-centered role.
β’ Reliable transportation.

β’ Experience with home health admissions and OASIS preferred.
