Admissions RN (Registered Nurse) – In-Home Intake & Assessment

As an Admissions Nurse, you focus on one patient at a time, completing thorough assessments without the pressure of juggling multiple visits or ongoing caseloads. Responsibilities
• Complete Start of Care (SOC) admissions in patients’ homes.
• Conduct detailed clinical assessments and OASIS documentation.
• Initiate care plans and ensure a smooth transition into services.
• Educate patients and families on what to expect moving forward.
• Coordinate with the care team to set patients up for success.
Requirements
• Active Minnesota RN license.
• Strong time management and independent work style.
• Comfortable working in a field-based, patient-centered role.
• Reliable transportation.
Nice-to-haves
• Experience with home health admissions and OASIS preferred.
