Virtual Primary Care Registered Nurse

About the position The Virtual Primary Care Registered Nurse (RN) at Mercy Virtual Care Center is responsible for providing virtual care to patients requiring ongoing chronic disease management. This role involves collaborating with a multidisciplinary team to build patient relationships, monitor vital signs, conduct virtual visits, assist in patient panel management, and deliver individualized care aimed at improving patient outcomes. The position is full-time, with a standard schedule of Monday to Friday from 8:00 AM to 4:30 PM, and there is potential for hybrid remote work after training. Responsibilities • Provide virtual care to patients with chronic disease management needs. • Build and maintain patient relationships. • Monitor vital sign trends and conduct virtual patient visits. • Assist in panel management of patients. • Deliver individualized care to improve patient outcomes. Requirements • Graduation from an accredited registered nursing program. • Active Missouri RN licensure required, with eligibility for licensure in other states. • Minimum 3 years of healthcare experience required for RN I; 5 years for RN II. • BLS Certification required. Nice-to-haves • Previous telemedicine experience. • Experience in internal/family medicine or care management.
