Virtual Hospitalist, Hospital at Home Program

Job Description:
Provide high quality, patient centric, cost effective in-patient level of care in the home setting. *Please note this is not a remote position* This is a virtual position based out of our Command Center in Charlestown, MA.
Ā Clinical outcomes are equal to or better than inpatient hospitalization.
Ā Work in a team environment (Physicians, NPs, RNs) in a virtual Command Center
Ā Medically Home significantly reduces total medicine expense from avoidable inpatient visits, post-acute care and readmissions.
Ā Improved patient experience.
Ā State of the art equipment at Command Center and patients' home.
Ā Patients have home infusion, meals/nutrient consults, pharmacy/medical mgmt., mobile imaging, laboratory and DME/oxygen.
Ā Integrated care with primary care providers, specialists & ancillary departments.
Minimum Qualifications:
Licensure, Certification, Registration:
Ā BC/BE Internal Medicine/Family Medicine/Geriatrics.
Skills, Knowledge & Abilities:
Ā Experience as a hospitalist or SNF a plus.
Ā Committed to providing high quality, evidenced based medicine to a diverse patient population.
Ā Excellent clinical and communication skills.
Ā Demonstrated ability to work in a team environment.
