Key Responsibilities:
Conduct virtual phone conversations to manage patients' healthcare needs and develop tailored care plans addressing physical, mental, and preventative health challenges.
Utilize cutting-edge technology platforms and Electronic Health Records (EHRs) to document patient visits and ensure seamless care coordination.
Coach patients on treatment plans, including nutrition and wellness strategies, to promote holistic health and wellness.
Connect patients with essential resources and prepare them for medical appointments to ensure comprehensive care.
Requirements:
Active Compact LPN License
Minimum 2 years of clinical experience, with care coordination experience preferred
Proficiency in Electronic Medical Records and Microsoft Office
Excellent communication and problem-solving skills
What We Offer:
Full-time position with a flexible schedule (40 hours/week, Monday - Friday, 8 am - 6 pm in the patient's time zone)
Monthly bonuses up to $525 and referral bonuses up to $1000
