Traveling LPC/LCSW
About Us: Eag-Us is a multi-state mental health organization dedicated to improving quality of life and mental health outcomes for older adults. We provide behavioral health services across skilled nursing facilities and long-term care settings, combining in-person clinical care with telehealth options as appropriate.
Position overview: We are seeking a traveling Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW) to deliver compassionate, evidence-based clinical services to elderly residents in nursing homes and other long-term care settings across assigned regions. The ideal candidate will combine strong clinical skills with flexibility for travel and facility-based work.
β’ Primary responsibilities
β’ Conduct clinical assessments, diagnostic evaluations, and ongoing mental health treatment for older adults in nursing homes and long-term care facilities.
β’ Develop, implement, and revise individualized treatment plans and goals, using culturally competent and trauma-informed approaches.
β’ Provide individual and group therapy, crisis intervention, behavioral management, and family/caregiver consultations as needed.
β’ Coordinate care with interdisciplinary teams including nursing staff, case managers, physicians, and facility administrators to support continuity of care.
β’ Maintain timely, accurate clinical documentation in electronic medical records (EMR) consistent with professional and regulatory standards (HIPAA compliance).
β’ Participate in facility credentialing and quality-improvement activities; support program development for geriatric behavioral health services.
β’ Utilize telehealth when appropriate to supplement in-person visits and extend access to care.
β’ Required qualifications
β’ Active, unrestricted LPC or LCSW license in the state(s) of practice, or demonstrated ability to obtain required state licensure for assigned regions.
β’ Clinical experience working with older adults, geriatric populations, or within nursing homes, skilled nursing facilities (SNF), or long-term care settings.
β’ Demonstrated competence in assessment, treatment planning, psychotherapy, and crisis management for geriatric mental health concerns (e.g., depression, anxiety, dementia-related behavior).
β’ Proficiency with electronic medical records (EMR) and documentation practices; strong written and verbal communication skills.
β’ Ability and willingness to travel regularly to multiple facility locations; valid driverβs license and reliable transportation (or willingness to use company-provided travel arrangements where available).
β’ Ability to meet facility credentialing requirements (background check, vaccinations/TB screening, and any facility-specific clearances).
β’ Preferred qualifications / nice-to-haves
β’ Certification or formal training in geriatric mental health, dementia care, or palliative care; experience with behavioral interventions for dementia.
β’ Experience with Medicaid/Medicare documentation processes and working within interdisciplinary long-term care teams.
β’ Training in evidence-based modalities such as CBT, Motivational Interviewing, or trauma-informed care.
β’ Bilingual proficiency or experience serving diverse cultural populations.
β’ Telehealth delivery experience and comfort using remote platforms.
