Aya Healthcare is seeking a full time 1099 provider to support a hospice and palliative care service through a telehealth focused model serving the Portland metro area, including southwest WA. This need is ongoing and is looking for consistent weekday coverage.

• 1099 coverage
• Hospice and palliative care focus
• Telehealth based practice setting
• Full time ongoing need
• One provider requested
Work Details
• Monday through Friday schedule
• Telehealth visits including consults and med consults
• IDT participation and CTIs
• EMR Epic
• Support staff available
Requirements
• Board certified or board eligible in FM or IM with strong hospice experience
• Active OR license required
• Active WA license required
• OR and WA DEA required and must be in hand
• BLS required
