Supervisor, HCS (Remote in MS)
About the position Responsibilities • Develop a plan of care for each assigned patient from admission through discharge. • Assess members for care needs and develop treatment plans with practitioners and providers. • Ensure quality member care is provided and monitor patient progress towards desired outcomes. • Coordinate efforts of team members to respond to patient/family needs. • Identify and resolve barriers that hinder effective patient care. • Coordinate medical services and community resource referrals post-discharge. • Assist in implementing health management activities in accordance with regulatory standards. • Supervise day-to-day operations and adherence to health management program protocols. • Assist in orienting and training staff to ensure program implementation and service excellence. • Monitor staff performance and assist with performance appraisals. Requirements • Associate degree or equivalent combination of education and experience. • 3-5 years of relevant experience in healthcare supervision or management. • Active, unrestricted healthcare-related license in California, with willingness to obtain licensure in other states. Nice-to-haves • Bachelor's Degree or equivalent combination of education and experience. • 5-7 years of relevant experience in healthcare supervision or management.
