About the position

We are seeking a highly-skilled, compassionate, and professional Home Health Aide (STNA or CNA highly desired | experienced HHA, CHHA, DSP, or PCA will also be considered) to join our home health agency and provide hands-on personal care and support to clients in Brookville, OH, metro Dayton area, and surrounding areas of Montgomery County. A successful Home Health Aide provides personal care services under the direction of the Registered Nurse or Therapist in their region. You will be assigned to specific clients by the Registered Nurse or other appropriate professionals and perform services for clients as necessary to maintain personal comfort and safety in their home.

Responsibilities
β€’ Personal Care & Activities of Daily Living (ADLs) including safe transfers plus mobility and positioning support
β€’ Support safe use of assistive devices (walkers, wheelchairs, lifts)
β€’ Nutrition & Meal Support
β€’ Homemaking & Environmental Support (IADLs) including light housekeeping, errands, and tasks required to maintain a clean, safe, and organized home environment
β€’ Community Engagement & Appointment Support, Coordination, and (in approved situations) Transportation
β€’ Basic Health and Safety Monitoring
β€’ Care Plan Implementation
β€’ Dementia & Behavioral Support (if applicable)
β€’ Pro-active observation and excellent electronic documentation
β€’ Client Rights, Dignity & Confidentiality
β€’ Total Care Team Collaboration
β€’ Ongoing Training & Certification Requirements

Requirements
β€’ Minimum of 1 year paid in-home care or caregiving experience (STNA/CNA certification in lieu of experience will be considered)
β€’ Successful completion of all agency-provided HHA/PCA orientation, skills tests and competency evaluations.
β€’ Willingness and timely compliance to maintain all ongoing Training & Certification Requirements
β€’ Active CPR + FA certification and documentation of both (+ proof/documentation) - or willingness to complete through HCN within 2 weeks / prior to hire
β€’ Willing and able to regularly perform all physical demands of home care (including lifting/carrying up to 30–50 lbs. occasionally).
β€’ Exceptional on-time and attendance record, and consistent availability aligned to your chosen schedule type (PT/FT/Hybrid)
β€’ Character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful
β€’ Willingness and ability to perform required physical demands (see below)
β€’ Consent + Ability to pass a State & Federal background check (including fingerprints)
β€’ Must be eligible to work in home and community-based settings (HCBS)
β€’ 18yo + active Driver's License / State ID + US Work Authorization
β€’ Reliable transportation to all scheduled visits, within full reach of stated travel radius and assigned clients

Nice-to-haves
β€’ Active STNAs, CNAs, and DSPs (DODD), including new grads
β€’ HHAs and PCAs with 1+ years caregiving experience
β€’ 1+ year paid home care experience preferred (agency experience a plus)
β€’ Medication Administration Certification is highly preferred
β€’ Experience with G-tubes / feeding support
β€’ Experience with Hoyer Lifts
β€’ Experience with Seizure protocols
β€’ Experience with Behavioral health or crisis response
β€’ High School diploma or GED
β€’ Active STNA (State Tested Nurse Aide) or CNA certification, or previous home health aide certification
β€’ Previous home health experience (1-3 years)
β€’ Familiarity with Medicaid waiver services
β€’ 45–60 min radius preferred
β€’ Hybrid (weekday + weekend) schedule
β€’ Long-term / 2+ year commitments
