About the position Responsibilities • Collect pertinent data and information relative to the healthcare consumer's health or situation. • Analyze assessment data to determine actual and potential diagnoses or issues. • Identify expected outcomes for a plan individualized to the healthcare consumer or situation. • Develop and modify a plan of care that prescribes strategies and alternatives to attain expected outcomes. • Implement the identified plan of care. • Coordinate care delivery and transitions in care. • Employ strategies to promote health and a safe environment. • Evaluate progress toward attainment of outcomes. • Engage in professional role activities, including leadership appropriate to education and position. • Communicate effectively in all areas of practice. • Collaborate with healthcare consumers and other key stakeholders in nursing practice. Requirements • Graduation from an accredited School of Nursing. • BSN or demonstrated BSN enrollment required with completion within three years of hire. • Current BCLS certification or must obtain within one month of hire. • Licensed and currently registered nurse in the State of Connecticut. • Clinical specialty certification highly encouraged. Nice-to-haves • Experience in Med Surg nursing is preferred. • Participation in the SPIRE clinical advancement program is encouraged.
