We’re seeking experienced Registered Nurses to join our care team as Care Coaches, helping patients manage chronic conditions and navigate their healthcare journey. What We’re Looking For: • Proven ability to manage approximately 75 inbound and outbound calls daily while maintaining quality patient interactions. • Backgrounds in case management, home health, discharge planning, population health management, or chronic condition management are highly desirable. • Comfort working telephonically and managing patient caseloads independently. • Strong clinical knowledge and excellent communication skills.
