Job Overview:

As a Remote RN Clinical Care Coordinator, you will play a vital role in managing the care of members with complex medical and behavioral needs, promoting improved health outcomes, and enhancing the overall quality of care.
Key Responsibilities:

Conduct comprehensive needs assessments to identify members' medical, behavioral, and socioeconomic needs
Develop and implement personalized care plans, leveraging evidence-based guidelines to address chronic health conditions, health promotion, and wellness
Collaborate with internal care teams, providers, and community resources to ensure seamless care coordination and support member self-management
Provide education, coaching, and advocacy to empower members and their families, ensuring their needs and choices are fully represented and supported
Proactively manage care transitions, including discharge planning and post-acute care coordination

What You'll Need:

Undergraduate degree or equivalent experience
Current, unrestricted independent licensure as a Registered Nurse in Tennessee
2+ years of clinical experience
1+ years of experience with MS Office, including Word, Excel, and Outlook
Reliable transportation and ability to travel up to 75% to meet with members and providers throughout Putnam, Jackson, and White Counties

Preferred Qualifications:

BSN, Master's Degree, or higher in a clinical field
CCM certification
1+ years of community case management experience coordinating care for individuals with complex needs
Experience working in team-based care, managed care, or a similar setting
