Job Overview

We are seeking a dedicated Patient Health Navigator to join our remote team, where you will be instrumental in guiding our members through their healthcare experiences. In this essential role, you will assist members in comprehending their care strategies, accessing vital resources, and navigating challenges related to their care. Your efforts will include conducting thorough assessments, educating members, and collaborating closely with healthcare professionals to ensure the delivery of necessary services.

Key Responsibilities
• Conduct health risk assessments for designated members and triage as needed.
• Deliver ongoing education, outreach, and follow-up to facilitate understanding and adherence to care plans.
• Assist members in procuring essential documents, tests, and services for medical appointments.
• Address barriers to medical appointments by coordinating transportation, durable medical equipment (DME), and translation services.
• Collaborate with primary care providers and care management staff to identify members in need of specialized care.
• Connect members with relevant community resources and support services.
• Maintain accurate records of all activities and ensure compliance with established standards.

Qualifications
• High school diploma or equivalent required.
• Minimum of two years' experience in a community health-related role (e.g., patient advocate, health coordinator) required.
• Associate's Degree and experience in a managed care setting preferred.
• Familiarity with medical terminology is beneficial.
• Valid California driver's license and reliable transportation are required.
