They are seeking a Medical Director for Transitional Case Management to lead and advance their transitional care programs, support frontline care teams, and drive strategic initiatives to improve patient outcomes during high-risk care transitions.ResponsibilitiesDrive the evolution of our transitional case management program, ensuring we deliver measurable outcomes for members navigating high-risk care transitions, including hospital discharge, post-acute care, and movement between care settingsServe as a clinical resource for our frontline care team, providing education, guidance, and direct collaboration to support quality care during transitionsLead and contribute to strategic initiatives across the organization, including developing and maintaining clinical protocols for care transitions, supporting efforts to reduce avoidable readmissions and emergency utilization, and representing Thyme Care in external meetingsSkillsAt least 3 years of clinical experience caring for oncology patients after fellowship trainingBoard certified in medical oncologyUnrestricted medical licenseWillingness to obtain additional state licenses as neededExperience with transitional care management programsExperience with care transition protocolsExperience with reducing avoidable readmissions and emergency utilization in an oncology or complex care populationSkilled in juggling multiple tasks and working under pressure without sacrificing organization in communications and documentationEffective listener and communicatorAbility to build rapport and excellent working relationships with members and colleaguesComfort with ambiguity in fast-paced environmentsDesire to learn how to use new technologiesExperience with video chatting, Google Suite, Slack, electronic health records, or comfort in using and learning new technologyAbility to identify and prioritize a member's needs and take action to address urgent needsBenefitsEquityBenefitsOther opportunities at Thyme CareCompany OverviewThyme Care is a healthcare company that provides oncology care management services.
