Remote Home Health Quality Assurance Specialist (RN/LVN/LPN)
OVERVIEW:
A growing Home Health Agency in the San Francisco Bay Area is seeking a self-disciplined and detail-oriented Remote Home Health Quality Assurance Specialist (RN/LVN/LPN) to join our team remotely. This is a flexible contract (1099) position, ideal for experienced RNs, LVNs, or LPNs passionate about delivering high-quality care through OASIS, chart, and clinical documentation reviews as well as training clinicians in the field on best practices in home health.

QUALIFICATIONS:
Graduate of an approved school of professional nursing, with current licensure in the practicing state(s).
At least three (3) years of clinical nursing experience required (Home Health preferred).
One (1) year of OASIS QA review experience required.
LVN/LPN candidates must have at least three (3) years of Quality Assurance experience in Home Health.
Proficiency in documentation, time management, and organizational skills.
Experience with Kinnser/WellSky or similar home health software is highly preferred.
OASIS certification is highly desirable.
RESPONSIBILITIES:
Conduct comprehensive clinical record reviews for clinical notes/charting, starts of care, resumptions, recertifications, and discharges.
Ensure OASIS assessments accurately reflect patients current health status.
Perform patient chart audits to identify and resolve documentation discrepancies.
Collaborate with the team to develop educational programs that improve documentation quality and compliance.
Maintain proficiency in CMS and industry best practices for OASIS standards.
