Your Role
Β Provide monthly calls and support to patients enrolled in the CCM program.
Β Utilize your nursing skills to educate patients and their families.
Β Monitor patients progress and work with the physicians to ensure quality patient care.
Qualifications
Β Active LPN license (CALIFORNIA)
Β Experience in geriatrics or primary care
Β Ability to sit and talk on the phone for long periods of time
Β System: We require you to have a Windows i5 (or newer) processor, and to have Windows Pro (not Home). This can be a laptop or desktop.
Β You live in any of the following states: NV, NM, AZ, CO, UT, WA, OR
The Pay
Β The California LPN pay ranges from $28.85 - $30.27 per hour
Β The pay is a base pay of $24.00/hr plus a bonus of $2.00 per call. On average our Care Coordinators complete between 16-21 calls per day. High performers may be eligible for a bonus up to $1000 per quarter.
