The Role

The Registered Nurse Case Manager team at AptiHealth is pivotal to the success of our Case Management and Medication Management programs and provides a backbone of support to our patients and our providers. The RN Case Manager will assist with assessing patient case management needs and implementing case management care plans. Additionally, the RN Case Manager will assist with implementing providers’ orders, administer and coordinate medications, procedures and special tests, and document treatment as required by company policy and local/state/federal rules and regulations as directed by the Clinical Director.

The RN Case Manager will order, interpret, and evaluate diagnostic tests to identify and assess patients' conditions. They will assess and evaluate patients' needs for, and responses to, care rendered. The RN Case Manager will also take live calls from patients calling into the front office with urgent medical/medication-related questions and triage these calls. This role will work closely with a multidisciplinary team within the AptiHealth Medical Group to provide high quality care to each AptiHealth patient.

Requirements
• Successful completion of an accredited nursing program, Bachelor’s degree preferred
• Currently licensed as a Registered Nurse in New York
• Current Certified Case Manager (CCM) certification preferred
• Experience in behavioral healthcare a plus
• Strong computer skills and experience with Electronic Medical Record systems
• Experience with Microsoft programs including Outlook and SharePoint Forms preferred
• Strong organizational skills
• Experience working with a multidisciplinary team

Responsibilities
• Complete Case Management Assessment and Case Management Care Plan
• Collaborate with care team to identify patients' needs and assist in development of plan to meet needs
• Document all patient interactions in care coordination note
• Triage incoming phone calls and electronic communications; providing information and direction to patients and escalating as necessary to the appropriate care team member
• Assist prescribers with medication prior authorization, coordination with pharmacy, medication refill requests, scheduling/rescheduling with assigned prescriber based on clinical need
• Review PMP and screen for controlled substance prescriptions
• Assess for appropriateness for virtual care upon review of PMP and communicate concerns to both therapist and Prescriber
• Assist with obtaining ROI for PCP and/or specialists, as indicated and coordinate patient care as required/requested by member’s prescriber
• Perform patient medical review and gather additional medical history information as necessary
• Enroll member in HIXNY upon obtaining consent
• Orient new patients to the patient experience and collect needed documentation and answer questions around AptiHealth’s Medication Management Program
• Care coordination with AptiHealth’s referring partners (e.g., PCP, MCO, specialists)
• Assess and evaluate patients' needs for, and response to, care and medication
• Provide referrals to community-based organizations to address gaps/needs and close the loop on referrals utilizing Unite Us, assisting with addressing social determinants of health (e.g., food insecurity, housing, benefits)
• Coordinate with internal and external care team members (e.g., PCP, medical specialists)

Job Type: Full-time / Hourly (40 hours/week)
Location: Remote - NY Capital Region resident preferred
Working Hours: Monday–Friday, 9 a.m.–5 p.m.
Pay Range: $32-$40/hr
The final pay-rate may vary based on qualifications and relevant experience.
