Registered Nurse Assessor - Field In Ocean and Burlington Counties NJ

Position Summary: The RN Field Assessor is responsible for face to face assessing with members to evaluate the medical needs of the member to facilitate the member’s overall wellness and help them obtain the services they need to thrive. Addressing requests for services such as adult medical daycare, pediatric medical daycare, personal care assistant, nursing facility custodial requests, personal preference program and MLTSS program enrollment. Minimum of 20 assessments per week dependent on need, may be more. Assessments take into account information from various sources to address all conditions including co-morbid and multiple diagnoses that impact functionality. Assessments include the member’s level of work capacity and related restrictions/limitations. Using a holistic approach assess the need for a referral to clinical resources for assistance in determining functionality. Consults with supervisor and others on team in overcoming barriers in meeting goals and objectives. Utilizes case management processes in compliance with regulatory and company policies and procedures. Utilizes interviewing skills to ensure maximum member engagement and discern their health status and health needs based on key questions and conversation. The RN Assessor will also be expected to mentor new hires on the team, once, a level of proficiency has been attained in their role. Requirements • Must possess reliable transportation and be willing and able to travel up to 75% of the time within Ocean and Burlington Counties. • Must be comfortable completing Field based assessments for all socioeconomic status and settings. • Mileage is reimbursed per our company expense reimbursement policy • Must hold an active unrestricted Registered Nurse license in the state of New Jersey • 2+ years clinical practice experience e.g., hospital setting, alternative care setting such as home health or ambulatory care required • Successful completion of the NJ Choice certification is a contingency of employment. (As per NJ Division of Aging Services, Office of Community Choice Options guidance, an exam score of 80% or higher is required, along with successful completion of the State training modules, and field mentoring component.”) • Must be able to work Monday - Friday, 8:00am - 5:00pm EST • Proficient in Multisystem Navigation: Comfortable using multiple monitors and systems simultaneously to streamline tasks and improve efficiency. • Effective Multitasker: Skilled at talking and typing concurrently while managing various responsibilities with focus and accuracy. • Basic Technical Troubleshooting: Able to set up and connect essential hardware (monitors, keyboards, etc.) and resolve common technical issues independently. • Digital Collaboration & Documentation: Experienced in electronic documentation and virtual collaboration tools to support remote teamwork and communication. • Highly Organized & Results-Oriented: Maintains a structured approach to work, ensuring tasks are completed on time and in alignment with performance metrics. • Metric-Driven Performance: Demonstrates accountability by consistently meeting or exceeding established goals and expectations.
