Home Health Occupational Therapists (OT) responsibilities include:
• Conduct assessments to evaluate patients’ physical conditions and functional abilities
• Develop individualized treatment plans based on assessment findings and patient goals
• Provide hands-on therapy including therapeutic exercises, functional activities, and self‑care skill training
• Educate patients and families on proper techniques, safety, and rehabilitation strategies
• Collaborate with interdisciplinary healthcare professionals to coordinate patient care
• Maintain accurate and detailed documentation of patient progress, treatments, and plan updates
• Adjust treatment plans based on patient response and progress toward goals
• Attend staff meetings and in‑service programs as needed

Requirements for the Home Healthcare Occupational Therapist
• Valid Occupational Therapy license to practice as an OT
• Knowledge of medical terminology, physiology, and anatomy
• Experience in home health or a clinical setting (preferred but not required)
• Strong communication skills for working effectively with patients, families, and interdisciplinary healthcare teams
• Ability to work independently and as part of a team
• Compassionate, patient‑centered mindset with a genuine desire to help others

Job Type: Per Diem

Pay: Varies Pay per visit based on Home Health experience
