For more than 20 years, we’ve provided high‑quality, home‑based primary care to older adults who are homebound, medically complex, or living with chronic conditions. As part of the Parker Advanced Care Institute at VNA Health Group, our interdisciplinary team — physicians, nurse practitioners, and physician assistants — delivers comprehensive in‑home medical care, from routine visits to chronic disease management, telehealth, diagnostics, and wound care. Our goal is simple: to ensure patients remain safe, comfortable, and confidently supported in the place they call home.Position Summary: The Physician is responsible for providing high‑quality, compassionate primary care to homebound patients, ensuring their symptoms are well‑managed and their quality of life is maximized. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
