Physical Therapist
The Home Health Physical Therapist plays a critical role in delivering personalized rehabilitative care to patients within their own homes, promoting recovery, mobility, and independence. This position requires assessing patients' physical conditions, developing tailored treatment plans, and implementing therapeutic interventions to improve strength, balance, and functional abilities. The therapist collaborates closely with patients, families, and interdisciplinary healthcare teams to ensure comprehensive care and optimal outcomes. By providing education and support, the therapist empowers patients to manage their conditions effectively and prevent further injury. Ultimately, this role contributes significantly to enhancing patients' quality of life while reducing hospital readmissions and facilitating community reintegration.As a Physical Therapist, you will:Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit. Applies concepts of infection control/universal precautions in performing patient careAssure continuity of quality patient care delivered with appropriate documentationProvide effective communication to patient/family, team members and other health care professionalsPromote agency philosophy and administrative policiesMaintain and complete all documentation within company stated guidelines.
