The Patient Services Representative plays a vital role in connecting patients and families to hospice and physician services, managing the intake process and ensuring timely access to care.ResponsibilitiesManage the full intake and referral process, from initial referral through admissionReceive, review, and convert patient and physician referrals in accordance with agency policiesAccurately enter, track, and maintain referral and patient data in company systemsServe as a key point of communication between patients, referral sources, clinical teams, sales, and leadershipAnswer inquiries about agency services and provide appropriate community resource referralsCoordinate referrals with supervisors and schedulers to support timely patient evaluationsMaintain organized systems to ensure continuity and quality of patient careProvide excellent internal and external customer service, including greeting visitors professionallyPrepare referral tracking and trending reports as neededCredential new physicians and facilities following established proceduresIdentify workflow gaps and recommend improvements to enhance efficiencyEnsure compliance with all federal, state, and local regulationsSkillsHigh school diploma or GEDInterpersonal skills and ability to communicate professionally and effectivelyKnowledge of medical terminologyStrong verbal and written communication skillsAbility to operate computer and office equipmentProficiency in Microsoft Office Suite and data entry skillsResponds to suggestion constructivelySkilled in conflict resolutionProven interpersonal, coordination, and leadership skillsReliable transportation with valid automobile insuranceAssociate's degree preferredMinimum 1 year of experience in a healthcare setting preferredKnowledge of Medicare, Medicaid, and third-party payor guidelines preferredBenefitsMinimum of 3 Weeks Paid Time Off (PTO)Company Vehicle ProgramFlexible Work ScheduleMentorship CultureMedical, Dental, and Vision Insurance401(k) with Employer MatchMileage ReimbursementCutting Edge TechnologyCompany OverviewOptimal Care, Inc.
