OHIO | Care Manager II, RN/LSW/LISW
About the position
Responsibilities
• Provide support to complex members for care coordination and case management services.
• Engage with members, caregivers, and providers to assess and plan individual member goals.
• Complete comprehensive person-centered assessments of physical, psycho-social, and environmental health.
• Identify problems/barriers for care coordination and implement appropriate interventions.
• Coordinate physical, behavioral health, and social services for members.
• Manage medications, including regular reconciliation and adherence support.
• Create and implement a plan of care to assist members in achieving health goals.
• Identify short- and long-term goals with associated time frames for completion.
• Document progress towards meeting goals and resolving issues.
• Schedule follow-up calls and make appropriate referrals.
Requirements
• Current and unrestricted Registered Nurse licensure in Ohio or licensed Social Worker (LSW or LISW) in Ohio.
• Bachelor's Degree (or higher) in a health-related field preferred.
• Minimum of 3 years of clinical practice experience with Medicaid populations.
• Minimum of 3 years of case management experience preferred in behavioral health.
• Proficiency in MS Office (Word, Excel, Outlook, Teams) and electronic medical record programs.
• Valid driver's license, car insurance, and reliable vehicle.
Nice-to-haves

• Familiarity with internet applications and documentation programs.
