Title: Lead Nurse Practitioner (NP) – Remote Care Model | $130K–$170K | Phoenix, AZ (Must be located in AZ)

We're working with a healthcare team focused on managing complex Medicaid populations through a telehealth-first, coordinated care model, designed to improve outcomes while reducing unnecessary hospital utilization.

The role
• Lead and coordinate care for complex patient populations

• Assist patients through transitions of care (hospital to home)
• Provide virtual care and guidance to reduce unnecessary ER visits
