Lead Licensed Vocational Nurse (LVN) - Home Health

Our client is seeking a dedicated and experienced Lead Licensed Vocational Nurse (LVN) for their Home Health division in **Dallas, Texas**. This hybrid role offers a unique blend of direct patient care and operational oversight, requiring flexibility and strong leadership skills. You will be responsible for providing high-quality nursing care to patients in their homes, including wound care, medication management, vital sign monitoring, and patient education. As a lead, you will also mentor and support a team of LVNs, assisting with care coordination, scheduling, and ensuring adherence to care plans and agency protocols. This position involves conducting initial patient assessments, developing individualized care plans in collaboration with physicians and other healthcare professionals, and performing ongoing evaluations of patient progress. You will also play a key role in training new staff, conducting field visits for quality assurance, and participating in interdisciplinary team meetings. Exceptional communication and interpersonal skills are essential for building rapport with patients and their families, as well as for effective collaboration with the wider healthcare team. Proficiency in electronic health record (EHR) systems is required for accurate and timely documentation. A minimum of three years of experience as an LVN, with at least one year in home health or a related field, is preferred. Prior experience in a leadership or supervisory role is a significant advantage. You must possess a valid LVN license in Texas and maintain current BLS certification. Strong understanding of home health regulations and best practices is expected. This role requires the ability to work independently, manage time effectively, and travel within the **Dallas** service area.
