Job Overview:
We are seeking a compassionate and skilled Health Aide or CNA to join our team at Fallon Health's Summit ElderCare in Webster, MA. As a Health Aide or CNA, you will play a vital role in providing personalized care and support to our participants, helping them to maintain their independence and quality of life.

Responsibilities:
Provide personal care, light housekeeping, and assistance with activities of daily living (ADLs) to participants in their homes and at the PACE center
Collaborate with the interdisciplinary team to develop and implement care plans
Carry out non-skilled treatments, including vital signs, transfers, and toileting
Maintain a clean and safe working environment and assist with meal preparation and serving
Provide accurate and timely documentation in the EMR and other systems
Requirements:
High school graduate or equivalent
Completion of an approved Home Health Aide or Certified Nursing Assistant Training Program preferred
Certification as a Home Health Aide or Nursing Assistant; CPR and First Aid Certification or willingness to be certified within 60 days of employment
One or more years of experience working with the elderly in a healthcare setting
Reliable transportation required
