Inpatient Transition of Care Coordinator
About the Role:
Join MedPOINT Management as an Inpatient Transition of Care Coordinator and play a vital role in enhancing patient outcomes. In this position, you will facilitate seamless transitions for patients from hospital to home, ensuring they receive the care they need. Be part of a dedicated team committed to making a difference in the lives of our patients in Sherman Oaks, CA. This is a full-time, temporary position.
Responsibilities:
⢠Coordinate post-discharge care plans and follow-up appointments for patients.
⢠Conduct comprehensive assessments to identify patient needs and barriers to care.
⢠Collaborate with healthcare providers to ensure continuity of care and medication management.
⢠Educate patients and families on discharge instructions and available resources.
⢠Monitor patient progress and adjust care plans as necessary to promote recovery.
⢠Document all interactions and care plans in the electronic health record (EHR) system.
⢠Participate in quality improvement initiatives to enhance patient care processes.
⢠Act as a liaison between patients, families, and healthcare teams to advocate for patient needs.
Requirements:
⢠Bachelor's degree in Nursing, Social Work, or a related field preferred.
⢠Current RN or LCSW license in California is required.
⢠Minimum of 2 years of experience in care coordination or case management.
⢠Strong understanding of healthcare systems and community resources.
⢠Excellent communication and interpersonal skills for patient interaction.
⢠Proficient in electronic health record (EHR) systems and documentation.
⢠Ability to work independently and manage multiple priorities effectively.
⢠Compassionate demeanor with a commitment to patient-centered care.
