Salary Range: $72,000 – $95,000 annually
Key Responsibilities:
Deliver telephonic or virtual clinical support to patients
Coordinate care plans with physicians, specialists, and other providers
Monitor patient health data and respond to alerts or symptoms
Educate patients on chronic condition management
Maintain accurate electronic health records in compliance with HIPAA
Qualifications:
Active RN license (must be valid in at least one U.S. state)
Minimum 2 years of nursing experience
Previous experience in case management, telehealth, or chronic care preferred
Strong communication, assessment, and documentation skills
Reliable internet connection and secure home workspace
