What You’ll Do As a Home Health RN, you will serve as the clinical leader in the home setting, responsible for: Performing comprehensive patient assessments (physical, psychosocial, functional, and environmental) Developing, implementing, and updating personalized plans of care Providing skilled nursing care for patients with complex and chronic conditions ‍ ‍ Educating patients and families on self-care, medications, nutrition, and disease management Evaluating patient progress and adjusting care plans as needed Communicating changes in patient status to physicians and interdisciplinary team members Ensuring strict adherence to infection control and safety standards ‍⚕️ Supervising and directing LPNs/LVNs and Home Health Aides to ensure safe, effective, and high-quality care delivery Monitoring clinical documentation to ensure compliance with regulatory and payor requirements Reviewing patient outcomes and ensuring continuity of care across the team Why This Role Stands Out Provide 1:1 patient care in a comfortable home setting Lead and support a clinical care team, including LPNs/LVNs Enjoy independence and clinical variety in your daily work Make a direct impact on patient independence and quality of life Collaborate with a supportive interdisciplinary team See meaningful, real-world patient progress every day What We’re Looking For Active and unrestricted Registered Nurse (RN) license Experience in home health, med-surg, or acute care preferred Strong assessment, leadership, and documentation skills Ability to supervise and delegate to LPNs/LVNs and care staff Compassionate communicator with a patient-first mindset Ability to work independently and manage multiple cases in the field Current RN License Valid Driver’s License CPR Certification Valid Auto Insurance At least 2 years of Home Health RN experience preferred HomeCare HomeBase experience preferred Benefits and Perks Medical, Dental, Vision Insurance 401k You’re eligible after 3 months of service/The plan is 100% fully vested immediately /Choice Health At Home contributes 100% of the first 3% you contribute each pay period Health Savings Account Life Insurance Short & Long Term Disability Insurance Paid Time Off and Paid Holidays Company vehicles could be available after 90 days of full time employment and all the terms and conditions set by the Fleet Car team are met.
