Our treatment programs were developed for families involved in the child-welfare system. Our overall clinical approach is grounded in the evidence-based standards for these populations. SAGE is a dynamic, growing, and friendly agency! We are actively looking for full-time Family Connections Consultants (FCC) to provide a variety of services to support SAGE clients, including conducting home visits, completing assessments with the families engaged in services, and developing service plans pertaining to goals set by DCS. This is an in-person position, but allows for some remote duties. This position allows for a flexible schedule, but must be available nights and weekends, as needed. Required qualifications:
• Bachelor’s degree of Social Work (BSW) or related behavioral health field
• A minimum of 1 year of human services work-related or internship experience
• Reliable transportation including proof of insurance and clean 5-year driving record

• Prior experience in In-Home behavioral health services
• Professional documentation and utilization of Electronic Health Record experience
• Bilingual
