**Essential Qualifications:**
- Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) in the state of residence.
- 2+ years of experience in medical social work or case management.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
