We are currently seeking an experienced Medical Social Worker to join our team in Chattaroy, WA, to provide per diem home health services to our patients. About the Role As a Medical Social Worker at blithequark, you will play a vital role in the interdisciplinary care team, providing emotional and social support to patients and their families. Your expertise will help patients cope with their illnesses or injuries, and you will work closely with other healthcare professionals to develop personalized plans of care. Key Responsibilities Conduct comprehensive assessments of patients' social and emotional needs, developing personalized plans of care to address these needs. Evaluate patients' relationships, financial resources, and available community resources, providing guidance and support to access these resources. Provide education and counseling to patients and families on coping with social and emotional responses to illness or injury. Collaborate with physicians and other healthcare professionals to develop and implement plans of care, ensuring that patients receive comprehensive and coordinated care. Participate in care coordination activities, acting as a resource to other healthcare team members in identifying and resolving patient needs. Supervise and evaluate the performance of Social Work Assistants, ensuring that all medical social services are provided in compliance with blithequark's standards and policies. Maintain accurate and timely documentation of patient interactions, including case conferences, progress notes, and other relevant records. Essential Qualifications Masters or doctoral degree in Social Work from a school accredited by the Council on Social Work Education. Current Social Worker licensure in the state of practice, if required by state law or regulation. Valid driver's license, auto insurance, and reliable transportation. Proof of current CPR certification. Minimum of one year of experience as a social worker in a healthcare setting, preferably in home health or hospice care. Excellent oral and written communication and interpersonal skills. Ability to read, write, and speak fluent English. Knowledge of medications and their correct administration. Ability to organize tasks, develop action plans, set priorities, and function under stressful situations. Preferred Qualifications Experience working with electronic medical records and other healthcare software applications. Knowledge of community resources and ability to access these resources to support patient care. Experience with discharge planning and ability to coordinate care transitions. Ability to communicate effectively with patients, families, and healthcare professionals at all levels of the organization. Current knowledge of healthcare regulations, standards, and best practices.
