We're seeking an experienced Licensed Social Worker to join our team in a remote capacity, working from anywhere in the U.S. As a Social Worker at CVS Health, you'll play a critical role in our health care case management program, partnering with registered nurse case managers to assess, plan, and advocate for members' benefit plans and health needs. About the Role This is a licensed Masters Social Worker position that requires a strong and diverse skillset in relevant areas to drive success. As a Social Worker, you'll work collaboratively with our clinical team to empower members to make positive lifestyle and medical choices. Your responsibilities will include researching available options and resources for members with financial, transportation, safety, and housing needs, all while working telephonically or via electronic exchange. Partner with registered nurse case managers to utilize a collaborative process of assessment, planning, and advocacy to meet members' benefit plans and health needs. Work telephonically or via electronic exchange with members across the entire lifecycle. Empower members to make positive lifestyle and medical choices. Collaborate with other clinical staff to assist members with community resources for medical, financial, and behavioral health. Essential Qualifications To be successful in this role, you must have: An active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in your state of residence. A Master's Degree in Social Work. 2+ years of experience in medical social work or case management. Preferred Qualifications While not required, the following qualifications can enhance your candidacy: Experience working in health care case management or a related field. Knowledge of community resources and services that support members' medical, financial, and behavioral health needs. Strong communication and interpersonal skills. Skills and Competencies To excel in this role, you'll need: Strong assessment and analytical skills to identify members' needs and develop effective plans. Excellent communication and interpersonal skills to work collaboratively with members, clinical staff, and other stakeholders. The ability to work independently in a remote setting, with minimal supervision. A compassionate and empathetic approach to working with members who may be facing challenging circumstances.
