Our PACE program provides older adults and their caregivers with a welcome alternative to nursing home care, offering comprehensive medical services and social support at a Summit ElderCare site while they maintain their independence in their own homes and communities. **Job Summary** As a Health Aide or CNA with Fallon Health, you will play a vital role in providing personal care, light housekeeping, and assistance with Activities of Daily Living (ADLs) to our participants in their homes and at our PACE center. You will work under the supervision of a Registered Nurse (RN) and collaborate with the Interdisciplinary Team to develop and implement care plans that meet the unique needs of each participant. Your expertise and compassion will make a significant difference in the lives of our participants and their families. **Responsibilities** * Provide personal care, light housekeeping, and assistance with ADLs to participants in their homes and at the PACE center * Assist with the development of care plans for participants through interaction with the Interdisciplinary Team * Collaborate with the Interdisciplinary Team to ensure appropriateness and continuity of care * Carry out non-skilled treatments, including vital signs, transfers, toileting, bathing, dressing, and other activities as assigned * Assist with individual and group programs, including planning, implementation, and evaluation * Maintain a clean and safe working and living environment in the PACE center and participants' homes * Assist with meal and snack preparation, serving, feeding, and clean-up * Use safe techniques in all interactions with participants * Provide accurate and timely documentation in the Electronic Medical Record (EMR) and other systems as required * Participate in carrying out schedule of daily cleaning and disinfecting of the PACE center in accordance with CDC guidelines * Actively participate in distribution of work for health aides to ensure care needs of participants and site tasks are completed timely and appropriately * Participate in regular team meetings for health aides at respective site * Perform all duties in accordance with Fallon Health and Summit ElderCare policies and procedures **Qualifications** * High school graduate or equivalent * Completion of an approved Home Health Aide or Certified Nursing Assistant Training Program preferred * Certification as a Home Health Aide or Nursing Assistant * CPR and First Aid Certification or willingness to be certified within 60 days of employment * One or more years of experience working with the elderly in a health care setting * Reliable transportation required **Benefits and Perks** * Competitive salary of $19-$23 per hour * Great benefits package, including: + 30 hours a week gets you our full-time benefits offering + Health Insurance offerings + Paid Time Off + 10 Holidays + 401K Savings Plan + Tuition Assistance + Vision Care, Life Insurance, and more! * Extra pay for shifts outside normal workday * Mileage Reimbursement * Respectful and desirable health aide/CNA work setting * Predictable hours * Safe and highly vaccinated work environment * Autonomy and flexibility in your role * Opportunities for career growth and professional development **Why Join Fallon Health?** At Fallon Health, we value our employees and offer a supportive and inclusive work environment.
