Job Summary

Key Responsibilities
Execute patient intervention pharmacy programs and ensure compliance with clinical pharmacy services
Drive pharmacy financial performance by implementing pharmacy programs to maximize reimbursements while improving patient health outcomes
Conduct patient consultations and provide clinical expertise to patients, healthcare professionals, and internal stakeholders
Review patient profiles and medication histories to ensure safe and effective medication therapy
Provide education and support to patients, healthcare professionals, and internal stakeholders on medication use and disease management
Collaborate with healthcare professionals to improve patient outcomes and reduce hospitalizations
Analyze data and clinical outcomes to identify areas for quality improvement and implement changes
Maintain current knowledge of pharmacy industry trends, regulations, and guidelines
Essential Qualifications
Bachelor's degree in Pharmacy or related field
Current pharmacist licensure as allowed by the relevant state licensing authority
Experience in patient care pharmacy programs and clinical pharmacy services
Strong communication and interpersonal skills
Ability to work independently and as part of a team
Proficient in Microsoft Office Suite, including Word, Excel, and PowerPoint
Preferred Qualifications
Experience with electronic health records and pharmacy management systems
Knowledge of insurance plans and reimbursement policies
Experience with quality improvement initiatives and patient safety programs
Certification in a pharmacy-related specialty, such as pharmacotherapy or medication therapy management
