Health Coach (Fully Remote)
About the position
Responsibilities
• Educate members on topics such as Managing Medicines, Nutrition, Safety, Benefits/Resources, Mindfulness, and working with Care Teams.
• Assist members in engaging with their healthcare providers and Aetna Care Management teams.
• Document all member interactions in the appropriate member records.
• Demonstrate empathy towards members and their caregivers.
• Troubleshoot resource unavailability and secure support to close member gaps.
• Follow program workflows and follow up with members to review personal action plans.
Requirements
• At least 2 years of experience as a Health Coach.
• Health Coach certification is required.
• Availability to support 8:00 AM - 5:00 PM Mountain Time or 7:00 AM - 4:00 PM Pacific Time.
Nice-to-haves
• Community health or public health experience preferred.
• Managed Care/Medicare/Medicaid experience preferred.
• Bilingual preferred.
• Bachelor's degree preferred.
