About the position

The Clinical Pharmacist (Value Based Care) position at Nemours Children's Health in Wilmington, DE, is a part-time hybrid role focused on providing clinical pharmacy services within the Value Based Care Pharmacy Team. The pharmacist will engage in initiatives aimed at cost reduction and enhancing patient care and quality outcomes. Responsibilities include direct patient interaction, medication reviews, education, and collaboration with healthcare providers to optimize medication regimens and therapeutic outcomes.

Responsibilities
• Provide clinical pharmacy services within the Value Based Care Pharmacy Team.

• Assist with pharmacy initiatives related to cost reduction and improved patient care.
,
• Conduct comprehensive medication reviews and provide education to patients and healthcare providers.
,
• Review and clarify patient medication regimens and make clinical recommendations to providers.
,
• Oversee daily activities of Certified Pharmacy Technicians and mentor pharmacy interns, students, and residents.
,
• Document pharmacy services provided in the Electronic Medical Record (EMR).
,
• Collaborate with the pharmacy team to analyze data and develop pharmacy initiatives supporting value-based care contracts.

• Participate in departmental program development and Continuous Improvement (CI) activities.
,
• Maintain compliance with professional policies and state and federal regulations.

Requirements
• Bachelor of Pharmacy or Doctor of Pharmacy (PharmD) required.

• Minimum of three (3) years of experience required.
,
• Current active pharmacist license in the State of Delaware required upon hire.
,
• Active Pennsylvania pharmacist license preferred, to be obtained within 12 months if not already licensed.

Nice-to-haves
• Value-based care and/or pediatric experience preferred.

• Medication Therapy Management Certification preferred.
,
• Board Certified Pediatric Pharmacy Specialist (BCPPS) preferred.
,
• Basic Life Support certification preferred.
