About the Role:
The Care Navigator provides ongoing proactive patient services through regular communication and personalized care plans for patients participating in the Chronic Care Management Program. They assess patient health status address concerns and facilitate access to community resources to improve overall well-being. Key responsibilities include patient education and support consistent patient check-ins detailed documentation and care coordination ensuring patients receive coordinated care across various healthcare providers such as PCPs specialists pharmacists and other healthcare services. Supervisory Responsibilities: This position has no direct supervisory responsibilities. Essential Duties and Responsibilities: Provide ongoing proactive services through monthly calls Review the patient's current health status and appropriately escalate concerns to the physician via the escalation protocols. (Follow the patient escalation protocols and workflows to ensure that the patient is receiving quality and standard care)Providing education related to medical conditions and factors impacting patient’s healthSetting health-related goals based on medical conditions & patient’s prioritiesIdentifying and assisting in closing clinical and non-clinical care gapsEmpowering patients to make sustainable health behavior changes to improve their well-beingContinually providing motivation encouragement and supportCommunicating with patient’s medical providers regarding important items such as new or worsening symptoms or medication access and adherence (per protocols)Sharing community resource options to help patients access the necessary resources at the right time (e.g. transportation food)Respond to patients providers and team leadersCreate update and maintain care plans using SelectPatient Management's proprietary softwareAchieve monthly call goals based on Supervisor Care Navigator advisory on assigned patient caseload. Typically the expectation is CNs are making 40+ phone calls and/or 11+ patients billed per dayMaintain productivity and quality metrics as definedAttend weekly team huddles to discuss organizational and team goals as well as strategies to best serve our patient populationSkills/Abilities: Empathetic and active listenerEffective time management skills and demonstrates the ability to maintain focus and productivity in a remote setting consistently meeting deadlines and performance goals.Maintain a strong sense of responsibility and accountability ensuring productivity and performance standards are consistently met in a remote environment.Engage fully with colleagues and teams remotely ensuring effective communication active participation and seamless teamwork.Effectively and appropriately communicate with doctors and healthcare professionals ensuring clarity professionalism and accurate information exchange to support patient care and coordination.Strong communication and interpersonal skills (written and verbal)Proficient computer skills including Electronic Health Record, Microsoft programs and Google SuiteAbility to work well independently in a fast-paced results-driven remote work environment Education and Experience: Certified or Registered Medical Assistant required2 or more years of experience in healthcare or a related field (3-5 years preferred)Family practice internal medicine and geriatrics office experience preferredAbility to work full-time (40 hours per week) Physical Requirements: Work is performed indoors with potential for exposure to safety and health hazards related to office work. Could periodically travel to other office and operational sites. The noise level in the work environment is usually moderate. Prolonged periods of sitting at a desk and working on a computer.
