Job Title: Remote Clinical Intake NurseLocation: RemoteJob Summary:We are seeking a detail-oriented and proactive Clinical Intake Nurse to join our team. This remote role is responsible for managing incoming referrals, coordinating with hospital partners, and supporting the development and maintenance of a strong and balanced patient census.Key Responsibilities:Monitor hospital referral portals daily to identify and respond to new patient referrals in a timely mannerCommunicate effectively with hospital case managers and social workers to coordinate patient admissionsReview clinical information to ensure appropriate patient placement and eligibilitySupport and maintain a healthy census mix aligned with organizational goalsCollaborate with internal teams to facilitate smooth admission processesMaintain accurate and up-to-date documentation of all referral and intake activitiesQualifications:Experience with clinical intake, case management, or admissions preferredStrong communication and relationship-building skillsAbility to work independently in a remote environmentProficiency with electronic medical records and referral portal systemsPreferred Skills:Experience working with hospital systems and post-acute care referralsStrong organizational and time management abilitiesCritical thinking and clinical assessment skills
