MUST RESIDE IN BELLEVILLE, IL WITHIN OR NEAR MADISON & ST. CLAIR COUNTIES
• Monday through Friday
• 8:00 AM - 5:00 PM CST
• Pay Range $35.00-$45.00 per hour
Job Description
Case Manager is responsible for health care management and coordination of members in order to achieve optimal clinical, financial and quality of life outcomes. Works with members to create and implement an integrated collaborative plan of care.
JOB REQUIREMENTS:
• Registered Nurse (RN), with THREE (3) years direct clinical care to the consumer in a clinical setting or Licensed Professional Counselor (LPC), or Licensed Master Social Worker (LMSW), which includes TWO (2) years of clinical practice to obtain their LPC or LMSW license.
• Current, valid, unrestricted license in the state of operations (or reciprocity).
• 3 years of wellness or managed care experience presenting clinical issues with members/physicians.
• Transitional care of experience
• Current IL driver's license, transportation, and applicable insurance.
• May conduct home, hospital, or LTC member visits.
• *All qualified candidates will undergo background and drug screenings.
