Provide personalized, compassionate care to members in their homes.
Utilize clinical expertise to assess and manage members' healthcare needs.
Communicate effectively with members, their families, and healthcare providers.
Work independently in a fast-paced environment to meet the needs of members.
Conduct thorough assessments of members' health status and develop individualized care plans.
Monitor and evaluate the effectiveness of care plans, making necessary adjustments as needed.
Collaborate with interdisciplinary healthcare team to coordinate and advocate for members' care.
Ensure compliance with all regulatory and accreditation standards.
Maintain accurate and timely documentation of all interactions and interventions.
Continuously strive to improve the quality of care provided to members.
Serve as a resource and mentor for other team members.
Stay up-to-date on current healthcare practices and guidelines.
Demonstrate a strong understanding of care management principles.
Handle sensitive and confidential information with professionalism and discretion.
Participate in ongoing training and development opportunities.
Adhere to company policies and procedures.
