Case Management Specialist (Remote)

About the Role:
Join the Aeetna team as a Case Management Specialist and make a real difference in the lives of individuals with complex healthcare and social needs. In this full-time remote position, you'll utilize your clinical expertise and compassionate approach to coordinate care, advocate for patients, and improve health outcomes.

Responsibilities:

Conduct comprehensive assessments of individuals' needs and develop tailored care plans.
Identify high-risk factors and connect individuals with appropriate resources and support services.
Organize and implement care plan interventions, monitoring progress and making adjustments as needed.
Collaborate with healthcare providers, managers, and clinical leaders to address barriers to achieving care goals.
Advocate for patients' rights and ensure access to necessary benefits and services.
Provide education, information, and support to empower individuals to make informed healthcare decisions.
Maintain accurate and up-to-date documentation of care activities.


Qualifications:

Minimum of 3 years of experience in a healthcare-related field.
Bachelor's degree or equivalent experience required.

Preferred Qualifications:

Certification such as CRC, CDM, CRRN, COHN, or CCM.
Experience with federal health insurance and Medicaid programs.
Experience with care coordination and health insights.
Experience working with geriatric, social health, and disabled populations.
Knowledge of evaluation, screenings, and care planning processes.
Bilingual proficiency (English/Spanish or English/Creole).


Compensation & Benefits:
This is a full-time remote position with a competitive salary ranging from $19.52 to $40.90 per hour, commensurate with experience.
