Bilingual Case Management Coordinator

Utilize fluency in multiple languages to provide support and guidance to diverse patient population.
Communicate effectively with patients and their families in their preferred language.
Collaborate with healthcare providers to ensure coordinated and comprehensive care for patients.
Conduct assessments of patients' needs and develop individualized care plans.
Monitor and track patients' progress, making adjustments to care plans as needed.
Provide resources and referrals to patients for additional support.
Maintain accurate and thorough documentation of patient interactions and care plans.
Communicate with insurance companies and other third-party agencies to ensure proper coverage and authorization for services.
Serve as an advocate for patients, ensuring their needs and concerns are addressed.
Stay current on healthcare policies and regulations to ensure compliance and quality of care.
Assist in coordinating and scheduling appointments and services for patients.
Collaborate with other healthcare professionals to ensure a multidisciplinary approach to patient care.
Demonstrate empathy, compassion, and professionalism in all interactions with patients and their families.
Maintain confidentiality and privacy of patient information.
Contribute to a positive and supportive work environment.
