Well-Being Advisor
Blue Zones Health is seeking a Well-Being Advisor to join our Well-Being Services team, supporting patients across affiliate PCP offices through a fully virtual, engagement-driven coaching model. This role is integral to integrating lifestyle medicine and behavior change strategies into patient care to improve health outcomes. The Well-Being Advisor will work within the Well-Being Services team, supporting patients at varying risk levels and guiding them in lifestyle interventions, SMART goal setting, and habit-building strategies aligned with the Blue Zones pillars.
This is a remote, part-time opportunity with hourly compensation between $28-$36/hour
Responsibilities:
Virtual One-on-One Health Coaching:
• Perform outreach to patients as needed to enroll in programming, follow up on missed appointments or answer questions outside of coaching sessions.
• Conduct initial and follow-up health coaching sessions.
• Guide members in setting SMART goals related to key wellness areas.
• Utilize motivational interviewing and behavior change techniques to support habit formation.
• Conduct coaching sessions with patients of varying risk levels; escalating patients of rising risk to the clinical team and coaching those who are also enrolled with case management
Virtual Group Coaching & Shared Medical Appointments (SMAs):
• Facilitate virtual group sessions and SMAs, focusing on chronic conditions and wellness goals.
• Foster a collaborative virtual environment for community support and shared accountability.
Collaborative Virtual Care & PCP Communication:
• Act as a virtual extension of the patient’s primary care team.
• Provide regular progress updates and collaborate closely with Care Concierges and medical teams.
• Participate in weekly huddles with the Well-Being Services team, meet with some of the provider groups and interdisciplinary teams to discuss patient needs and exchange necessary information
Qualifications:
Education:
• Bachelor’s degree in Health Coaching, Nutrition, Exercise Science, Public Health, or a related field (or equivalent experience).
• Health Coaching Certification (NBC-HWC preferred) or equivalent.
Experience:
• 2+ years of experience in virtual health coaching, behavioral change interventions, or lifestyle medicine coaching.
• Proficiency in telehealth and virtual engagement platforms.
Preferred Qualifications:
• Experience in Medicare Advantage, ACO, or managing chronic diseases.
• Background in lifestyle medicine or chronic disease self-management.
Skills:
• Fluency in Spanish (written and verbal) is required
• Strong patient-centered communication and motivational skills.
• Proficiency with digital tools and data-driven goal setting.
• Ability to collaborate across multidisciplinary teams and adapt coaching methods to patient needs.
Performance Metrics:
• Engagement and retention rates.
• Improvements in chronic condition management and lifestyle health.
• Value-based care alignment
• Member satisfaction and feedback on the coaching program.
