In this remote role , you will review hospital clinical documentation to determine if patient meets criteria for admission to a skilled nursing facility. Your assessment will ensure timely, accurate, and compliant admission decisions that support high -quality patient care and optimal utilization of services.QualificationsQualifications:Current, Active RN or LPN license in good standingMinimum of 2 years of clinical nursing experienceStrong knowledge of Medicare and Insurance preferredExcellent verbal and written communicationProficient in reviewing electronic medical records( EMR)Self - motivated with strong attention to detail and the ability to work independently in a remote environment.Compassionate, patient-focused approach to healthcare.Work Environment:RemoteRequires high-speed internet and secure workspace.Compensation:Hourly rate based on experienceResponsibilitiesKey Responsibilities: Review hospital medical records, clinical notes, and supporting documentation for patients referred to skill nursing.Apply Medicare and insurance - specific criteria to determine eligibility for SNF( Skilled ) admission.Communicate findings and clearly to hospital discharge planners, admissions teams, and other healthcare partners.Document review outcomes accurately in the required systems and formats.Collaborate with admissions and clinical teams to clarify criteria. Address discrepancies ad support seamless patient transitions.Maintain compliance with HIPPA and organizational privacy policy.Serve as a liaison between patients, family members, and healthcare providers.
