Virtual Patient Manager
About the position
The Virtual Patient Manager at CVS Health is responsible for overseeing the day-to-day operations of post-acute care performance networks. This role focuses on managing relationships with skilled nursing facilities (SNFs) and home health agencies, ensuring effective patient transitions and stability post-discharge, and leading initiatives to improve care performance and efficiency.
Responsibilities
Ā Manage and develop relationships with SNF and Home Health Agencies

Ā Communicate and collaborate with internal client team
,
Ā Engage with Skilled Nursing Facility Interdisciplinary Teams to achieve appropriate length of stay for ACO and Value Based Care patients and ensure patient stability post-discharge
,
Ā Coordinate and lead monthly meetings with leadership at each performance network facility/agency to report outcomes and recommend process improvements
,
Ā Identify high priority care redesign interventions to improve performance and efficiency of facilities within local network
,
Ā Work within CVS' patient tracking software platform daily
,
Ā Collaborate with Clinical Strategists and Site of Care Manager to communicate PAC interventions to partners
,
Ā Confirm start of care date for HHA including any documentation of any delays in start of care or if readmitted
,
Ā Review and initiate SNF escalations for patients with SDOH or Clinical barriers to DC
,
Ā Review readmission reasons and initiate process improvements with SNF/HHA
Requirements
Ā 6+ years experience in healthcare and/or business
,
Ā Strong critical thinking needed to analyze medical data and reports
,
Ā Patient assessment and evaluation experience, ability to exercise independent judgment
,
Ā Understanding of Care Transition Models (Naylor, Coleman, Boost, etc..) is a plus
,
Ā Case management and discharge planning experience are preferred but not mandatory
,
Ā Bachelor's degree required
